The Player can probably undress the college student with the gentle caress of his mouse icon... That is, my good sir, quite obviously a girl.  Note the breasts.  And she's obviously remarking on how "boring" it is for the main character to have cardiac arrhythmia, instead of some other not visibly obvious disability that would have him attending a school for children with disabilities.

Also, dating sims don't work like that.  The sex sims do, for what its worth, but this is a dating sim.
