Big things for me would be the Races, Feats, PrCs, and maybe some items spells. Although Getting the setting down comes first.

my biggest kvetch that needed to be solved was what I call  'Becoming a better thief by killing things'.  I had to go to a skill-based system because of of that...

The homebrew I am cooking up at the moment is most likely going to leave the rule system up to the individual gamemaster, though it'll have a few tips on using common systems. However, when I am happy with it to the point of wanting to run it, I would either go with d20 Modern or nearly-freestyle play.

Eventually, i'll want to devise my own system. A goal-oriented magic system (i.e., the player declares what they want to do, then an appropriate penalty or reduction in something - mental strength, or physical constitution, for example - is levied against that player by the GM) seems to be rare these days in RPGs.
