Most use either Adobe Photoshop or GIMP. The former costs a fair amount (but it's worth it) while the latter is free to download and use (though like all open source programs it takes a bit of getting used to).

Gamer Printshop

This map is not yet complete (and I don't have a thumbnail yet), but this is my entry (so far) for the One Page Dungeon Contest (for prizes.) This is concurrently my entry for this month's challenge at the Cartographers' Guild. The thread is titled Necromancer's Lair, but I've since tweaked that idea to be the lair of a Lich. This part of the map is closed off from the rest of the dungeon, accessed via an A-B Portal. The B-end shown as the magic circle at the base of the stairs in the Blood Canyon chamber (far left) is activated automatically by evil beings without mishap. However, neutral and good aligned beings must fail as Fortitude check DC 25 to pass through. If they fail their save which activates the portal, neutrals lose a single life energy level, while good folks lose two levels. This makes it extra nasty to enter the inner sanctum.

Also see the skeletal dragons surrounding the Skull Altar - the skull's open mouth is a direct portal to the Negative Energy Plane. The altar is surrounded by a moat of blood.

Gamer Printshop

I almost thought about not doing this due to the amount of work involved, but I bit the bullet and gave it a shot. I created a detailed Boneyard, as part of my One Page Dungeon Contest entry. This took a better part of four hours to hand-draw, scan, import to Xara Xtreme Pro 4.0, then apply colors, beveled shapes and drop shadows. I hope to never have to do this again, but I'm satisfied with the results!

Wow looks like you went through a lot to get that map completed. Well, let me tell you, your effort was not wasted! It looks great (as does the pic quoted from above!).

One Page Dungeon Contest map entry is complete!

Notice the webpage link at the bottom of the legend - it doesn't exist yet, but I will be doing this tonight. This map is completely hand-drawn, scanned, color/bevel/shadows done in Xara Xtreme Pro 4.0 - got almost 20 hours in this map, probably my most time consuming map ever.
