I like the Sea-Tarrasque. I also love the idea of Vampirates. They are vampires, but also pirates!

Hmm, i'll second haunted ships. Also, curses and great hidden treasures. Maybe some kind of Bermuda Triangle? And even if fantasy, you should probably include low-level guns and cannons... The typical "hidden islands" that can only be found if you have certain items or at a certain point of year might also be useful. Talking of items, strange magical stuff like maps, compasses, cutlasses, eyepatches and the stuff might also be cool if it's a DnD game. Can't come up with any more yet ^^

(Also, you might want to take a look at China Mieville's "The Scar". The book is set on the floating pirate city of Armada. Also, there is an area in Eberron called the Principalities which are largely focused on pirates)

I really like the idea of weird treasure, things that are inherently magical but maybe nor inherently useful. For example, a Pearl the size of your head that has a soft but steady heartbeat.

I also remembered that old cartoon, 'The Pirates of Dark Water'. I liked it. A pity they never ended it.

Crazy storms. At night. Waves crashing over the sides of the ship, and there are sharp, jagged rocks ahead, et cetera. Perhaps even supernatural storms, called down by salt-encrusted old shamans on rocky cliffs, bent on driving ships to their doom.

Don't forget The Law! Pirates ought to be chased by navy ships and privateers, bent on bringing them to justice (perhaps at the bottom of the briny deeps.) It might be fun to build up one or more such ships as recurring enemies-- a tough ship or a zealous captain can be a memorable foe-- perhaps better-armed but slower than your players' ship, encouraging them to continuously slip out of their grasp, past blockaded bays and dire straits.

An East India Company kind of organization (almost its own sovereign power and barely above being licensed pirates themselves)and a Madagascar-like island for the pirates to refit. Also, have at least one floating city created by dwarves or gnomes and protected by ironclads...its too cool not to have
